The pricing is per screw hole to extract.
Our process for removing screws is typically guaranteed for screws larger than #6. For screws smaller than #6; for example, #4 or #5 screws, the chance of success decreases. Once the screw is removed we chase the threads to ensure they are cleaned up.
If a screw extraction fails, most of the time the hole can just be threaded a size or two up.
